The Government of India’s Free Diagnostic Initiative has emerged as a transformative step towards improving healthcare accessibility. In alignment with this vision, the Madhya Pradesh government implemented a groundbreaking model for pathology services four years ago. This initiative has revolutionized diagnostic services in the state, positively impacting millions of lives.

Under this model, citizens can access over 100 diagnostic tests free of cost at locations near their homes. Leveraging innovative frameworks such as the Wet Lease Reagent Model and the Hub and Spoke Model, Madhya Pradesh has established a robust network of laboratories and collection centers that deliver efficient, high-quality diagnostic services to even the most remote areas.

Key Features of the Model

A) Infrastructure Highlights

  • 85 High-End Reference Labs: Established under the Wet Lease Reagent Model and equipped with advanced diagnostic machines.
  • 324 Hub Labs: Located at Community Health Centers (CHCs), forming a vital part of the Hub and Spoke framework.
  • 1,690 Collection Centers: Strategically positioned at Primary Health Centers (PHCs) for convenient access.
  • Seamless Connectivity: A fully integrated 360-degree network ensures smooth sample collection, processing, and delivery of results.

B) Workflow and Operations

  • Doctor’s Prescription: Tests are prescribed at PHCs after a preliminary medical examination.
  • Data Entry: Patient information is entered into a digital portal.
  • Sample Collection: Samples are collected in barcoded vacutainer vials, ensuring traceability.
  • Sample Transportation: Following strict protocols, trained runners transport samples to the nearest hub lab.
  • Advanced Testing: High-end diagnostics are conducted using USFDA-approved machines.
  • Report Delivery via SMS: Medical Reports are uploaded to the LIMS software and sent to patients via SMS, ensuring privacy and promptness.

C) Diagnostic Capabilities

  • Routine tests performed at PHCs.
  • Advanced tests (e.g., cancer markers, thalassemia) conducted at district/civil hospitals under NABL guidelines.
  • Equipped with fully automated analyzers for biochemistry, hematology, coagulation, immunoassay, urine analysis, and HPLC testing.

Wet Lease Reagent Model

This model optimizes resource utilization by leasing reagents and diagnostic equipment rather than purchasing them outright. Under this model:

  • Private companies, reducing upfront costs for the government, provide the equipment.
  • The leasing companies, ensuring uninterrupted functionality, handle regular maintenance and calibration.
  • This cost-effective approach has allowed the state to establish 85 high-end reference labs equipped with state-of-the-art technology.

Hub and Spoke Model

The Hub and Spoke framework ensures efficient service delivery across all regions:

  • Hubs: Serve as central processing labs, equipped with advanced diagnostic machines.
  • Spokes: Include PHCs and CHCs that act as sample collection and initial testing points.
  • This interconnected system ensures faster sample transportation, timely test processing, and quick result dissemination.

  • Accessibility: Citizens, even in remote villages, can access diagnostic services within a 7 km radius.
  • Affordability: All tests are free, eliminating financial barriers to quality healthcare.
  • Efficiency: Automated systems and seamless integration ensure swift sample processing and result delivery.
  • Transparency: Digital billing and live data tracking uphold accountability.
  • Quality Assurance: NABL compliance and USFDA-approved equipment guarantee accurate and reliable results.
  • Patient Privacy: Secure SMS delivery ensures confidentiality.
  • Community Impact: Over 8 crore tests have been conducted, benefiting 2 crore individuals in the past four years.
